To ensure patient safety, RN Lena makes a fall risk assessment and implements fall preventive measures for the patient.) Lena: Hello, my name is Lena. I'm your RN for this shift. May I check your name Mrs. Peterson (Showing her armband to Lena.) ena: Thank you. What's your name and date of birth? (Looking at the information on the armband.) Mrs. Peterson ena: Good. It is nice to meet you, Mrs. Peterson. Can you tell me what happened and why you were admitted to the hospital? Mrs. Peterson ena: I'm so sorry to hear that. Did you lose consciousness? Do you remember what you were doing when you fell? Mrs. Peterson Lena: Where did you get hurt? Mrs. Peterson Lena: Oh, yes. Lena performs an assessment. The X-ray you had when you were in the emergency Department shows that you right hip is fractured. Dr. Baker believes it will heal without surgery. But since you had cardiac surgery 6 months ago, he wants you to stay in the hospital for observation. After Dr. Baker runs tests and reexamines you, if he thinks it is safe for you to go home then you will be on your way. All right? Mrs. Peterson

From the time you were brought into the hospital till now have you Lena: Thats right. It will take some time for your hip fracture to heal since your hip fracture has not been stabilized surgically. Physical Therapy will come to do an evaluation and help you when it is OK for you to start walking again Mrs. Peterson. Lena: Now, do you have any problems with elimination? Mrs. peterson Lena: How many times do you usually go to the bathroom at night? Mrs. Peterson Lena: Are you on any diuretics, like Lasix Mrs. Peterson. Lena: Do you take any blood pressure medicine? Mrs. peters Lena: After the doctor says it is Ok for you to get out of bed, be careful when you get up quickly. Your blood pressure medicine might make you feel dizzy and you might even fall Mrs. Peterson Lena: Let me check the bed to make sure the bed is locked. Yes. it is locked and the head of the bed is at its lowest position. The bed side rails are up. The bed is very well secured. Here is the call-light. Press the button if you need any help. I or somebody else will come to help you, OK? Mrs. Peterson
